Dr. Frank Choate

- Dr Choate is a licensed psychologist and owner and founder of Decisions Pschological Services, PC. He has been granted Health Service Provider in Psychology status. He has over 17 years experience in the therapy field. Dr. Choate is a state certified (IAAP) addictions counselor and nationally credentialed as a Master Addictions Counselor (NAADAC). His specialty is psychological assessments.
- He is a member of The American Psychological Association, The Indiana Psychological Association, The National Association of Alcohol and Drug Abuse Counselors, The Indiana Association of Addictions Professionals, The Society of Personality Assessment and The International Rorschach Society.
Ms. Kiley Caughell
MSW, LSW
- Ms. Caughell graduated from Indiana Wesleyan University with a bachelor's degree in psychology and a minor in addictions counseling. She obtained her Master's degree in social work (MSW) from Indiana University, where she graduated with honors. During her studies, she gained extensive clinical experience by working in an inpatient/outpatient behavioral health facility in Bloomington, IN. Ms. Caughell has since become a licensed social worker (LSW) who enjoys working with all ages, especially adolescents and young adults. She has an exceptional ability to connect with these age groups which greatly enhances her effectiveness as a therapist.
Mr. David Walker
MS, LCSW

Mr. Walker brings a unique path of professional expertise in working with troubled youth. In his 25+ years of social work, he has worked as a therapist, house parent, program coordinator and program director, and special needs educator. He has worked at the former Koala Center, Kingwood and Niequist Center. For several summers, he worked with youth summer camps at Bowen Center. For the last 14 years, he has worked with the JESSE program.
Ms. Chloeann Choate
MPA, Psychology Technician
- Ms. Choate is Co Founder of Decisions Psychological Services, PC. She has more than 10 years experience in the disability field and is considered an expert in the rules of federal and state programs regarding employment for individuals with disabilities. Using her knowledge of the work incentive rules to assist those on Social Security disability benefits, she helps families be aware of the rules when they go to work. She also helps young adults assess their vocational strategies, as she oversees the Career Assessment segment of our Center.
Ms. Teri Carr
- Ms. Carr is the office manager of Decisions Psychological Services, PC. She has an extensive history of social service expertise. After managing the local medical office of the late Dr. Jamie Ramos, she became a board member of the First Steps Early Intervention Council. Ms. Carr also spent over ten years as Fulton County Step Ahead coordinator. As Associate Director of the Fulton County Community Foundation, she helped develop many lasting relationships within the philanthropic realm. Ms. Carr is one of three volunteer co-directors (and founders) of CAMP-WE-CAN, a unique camp opportunity for school-aged youth with special needs.
